Readmission Watch

Our new Readmission Watch page is designed to provide you with an overview of admission, re-entry, and discharge assessments so that your facility can monitor acute care readmissions that may impact your standing as a provider of choice.

Readmission Watch allows you to monitor acute care readmissions; track rate of re-entry assessments, review admissions and discharge assessments, review length of stay between admissions and discharges, identify residents at risk, and monitor diagnosis trends.

Similar to other Watch pages in CareWatch, Readmission Watch consists of a series of portlets that display varying information based on admission, re-entry, and discharge assessments. The page can be “customized” for each user as she/he has the ability to select which portlets are displayed and can also select the placement of the portlets on the page.

Examples of Readmission Watch portlets include: Discharge to Acute Care Facility after Entry to Facility; Readmission to Acute Care Facility – Discharge Diagnosis; and Multiple Readmissions from an Acute Care Facility for “Episode of Care.”
