CareWatch® and RiskWatch® – (January 2011)
We have added an HL7 interface to CareWatch and RiskWatch that will make life easier for you. It allows you to send us ADT (Admission, Discharge, Transfer) records. This new feature provides the following advantages:

- New admissions (ADT-A01 admission message type) will be registered in the system and added to the Assessments Due schedule in CareWatch. Also, new residents will become available for incident reporting in RiskWatch.
- Discharges (ADT-A03 discharge message type) will likewise be registered in CareWatch and the Assessments Due schedule will notify you when a discharge assessment is needed.
- Resident status changes are reflected in CareWatch and RiskWatch within minutes of submission.
- New admissions (ADT-A01 admission message type) will be registered in the system and added to the Assessments Due schedule in CareWatch. Also, new residents will become available for incident reporting in RiskWatch.

Identify the customer implementation contact information. The flexibility of the HL7 messaging standard means that each implementation will require some refinement.
- Identify the customer contact once HL7 processing is established. eHDS monitors message traffic to identify disruptions in message traffic. If we have not received a message in the past 24 hours, our support staff will need to know who should be contacted (regular business hours only).
- Define the translation maps for facility identifiers, rooms and units. The customer implementation contact, in conjunction with the eHDS support team, will define the identifier maps between the customer’s system and the eHDS system.
- Create a secure FTP drop site to receive messages. eHDS uses a secure file transfer to receive files containing one or more HL7 messages. This task is done by the eHDS support team.
- Construct HL7 files and create the transmission mechanism. The customer must create the HL7 files (one or more per document) and create a mechanism to send those files to our secure site on a defined schedule. The files must be simple text documents (not zip files) and must have the file extension .hl7.
- Create a message channel to read the HL7 files. eHDS support staff will create an HL7 message channel which will read the files from the secure FTP folder and process the messages into CareWatch and RiskWatch.

Notes:
~ The eHDS implementation of HL7 supports only Admissions (ADT-A01) and Discharges (ADT-A03) at this time. Support for any additional message formats will require system enhancement.
~ The eHDS implementation does not provide message acknowledgments.
CareWatch Updates
Thursday, December 9th, 2010 by MonikaCongratulations–we all made it through the MDS 2.0 to MDS 3.0 transition! Throw in our product transition to a new CareWatch format and things have been pretty crazy the last few months. We are still hard at work fine tuning the details of the new CareWatch look and we greatly appreciate your patience and support during this process. Please keep your feedback and suggestions rolling in…your input helps us make CareWatch the best it can be.
Recent updates and tweaks to the system include:
RUG Potential Page
-added new summary table at bottom of report, “Potential (excluding negative values)”
-added “Always exclude this rule for this resident” as an option on the rules triggered pop up window
-updated the rule processing regarding ICD-9 codes
-changed the “Start” and “End” date text boxes so that they accept single digits for the day and/or month
-fixed an issue regarding fields I8000 (A-J) in RUG 34
-adjusted the summary table totals so that they calculate when a comma is present
-various fixes to some of the drop downs and columns not displaying correctly
-added resident count to the totals for the category summary table
Assessment Due Page
-a Significant Events assessment will restart the comprehensive yearly timer for assessments due
-PPS assessments more than 14 days overdue will no longer affect the list
-unscheduled PPS assessments (where A0310B = 07) are now excluded from the assessment due calculation
-the transmission date has been added to the assessment due table
-transmission date has also been added to the assessment due summary
Suspended Assessments
-added suspended assessment summary portlet as an option for the Welcome Back page with drill through pages
MDS Details Page
-implemented fix so that when you use the sort and filter function on the page, the page keeps those conditions
ADL Watch
-added sorting capabilities to the resident lists which are drill throughs from the ADL Watch page
-corrected issue with short stay assessments not being identified on the ADL Watch pages
MDS 3.0 Assessments Page
-added fatal and warning edit counts
-added logic flag counts to the page
-added “other” error counts to report
-included CareWatch assessment ID in the report
Medicare Resident Roster
-added several new columns of data and removed others
-renamed some of the columns to give more precise meaning
-limited the earliest report date for the page to October 1, 2010
-made some amendments as to which assessments are included in the report
-translated MDS 2.0 assessments that are missing A1600 dates now display “N/A” as the admission date
Transmission Watch
-if an assessment has already been transmitted and another transmission is attempted for the same file (no changes made it), the assessment will throw an error and be marked ”invalid” so that you know it has already been transmitted.
-you can now request a report from CMS through the CMS Reports portlet on the Transmission Watch page. Click the link “Request a Report” to do so.
NOTE: Currently only MDS 2.0 reports are available. CMS has disabled the MDS 3.0 reports for reasons unknown to us.
As soon as they are available, you will be able to request MDS 3.0 reports from this portlet
-new functionality has been added to the Manage Facility pages for administrators–you can now add an email address for email notifications regarding MDS transmissions–can e notified if assessment is rejected by CMS
-added a function in the “Assessments Transmitted” and “Assessments Rejected” portals which allow a user to download the actual file that CareWatch transmitted to CMS. This link appears under “Action” when the file is in the pending stage.
Miscellaneous
-a bug has been corrected regarding the Medicaid number and resident identification for incoming assessments
-updates have been made to several logic flags
-several administrative pages have been added including User Access to a Facility; Facility Access to a User; and Manage Users enhancements
-new columns were added to the MDS 3.0 Submission History page which show counts for the various processing statuses of individual MDS files
-added new page called “Resident Information” which can be found in the “Reports” drop down menu
-added pages to Quality Watch menu: QM Benchmark Portlet; QM Benchmark page; and QM SPC charts
Bring on MDS 3.0!
Friday, June 11th, 2010 by MonikaNot only will we be ready for MDS 3.0 on October 1, but we will be ready with a new look and new functions for CareWatch.
We have been listening in earnest to your ideas and requests for additional functionality in CareWatch, and with the MDS3.0 rollout, you will reap the benefits of our labor. We have an updated look for CareWatch that is more intuitive to use, easier to navigate, and features customizable options for each user. We even have an option to compile your MDS assessments directly into CareWatch with our new data entry tool.
The dashboard has been re-designed to allow you to create “portlets” of information that you want to see immediately upon login such as the number of logic flags in recently submitted assessments, the assessments due list, and Favorites where you can create a list of links to the reports that you use most often. The portlets can also be easily rearranged physically on the page and edited any time you desire. Here’s a snapshot of the new Welcome Back page:

In addition to experiencing less clutter and easier navigation in the system, we have added the ability for user administrators to define their own roles for users. They can now customize and create roles that meet precisely the needs of a specific user. For example, if an organization wants Mary MDS to be able to submit MDSs and to view use analysis of the site, the administrator can give her both of those permissions. At the same time, the administrator may want Annie AssistantMDS to only be able to submit MDSs and so her role can be limited to just that. This provides each organization with the ability to precisely manage their staff resources and operate at the level that is most efficient for them.
And, finally (not really, but we do want to have some things to surprise you with later), the password system has been changed so that if a user forgets her/his password, they can retrieve it or change it themselves! I know you’re doing the happy dance now–so are we! We have implemented a security system very similar to many sites that folks are already used to which asks for and logs a security question and answer. Now, when users forget their login passwords, they can answer their security question and get themselves back into the system. No more trying to hunt down your security person to reset your password…life is good!
As you can tell, we have lots of new and exciting things in CareWatch for 3.0, the items listed above are just a few of them. Rest assured that all of the things that you have come to rely on with CareWatch are still there: the ability to increase your data accuracy with the eHDS logic flag system, your data analyses, your Watch pages, data integrity reports, reimbursement reports, etc. All of these are still there, but with the added bonus of a site that is easier to navigate, easier to use, more efficient, and integrates more smoothly into your daily work flow. And don’t forget, you can submit your assessments directly through CareWatch now and retrieve your validation and CASPER reports through us too–we are your comprehensive MDS3.0 solution.
Watch for future bulletins, emails, or other communication regarding training on CareWatch and MDS 3.0, including the opportunity to use our demo site to start practicing with the updated tools in July.

RUG Payment Analysis Potential
Saturday, June 5th, 2010 by EveFor those of you who live in states that calculate Medicaid reimbursement using RUG 34, we have developed a new set of payment analysis pages that allow you to see the impact of possible missed opportunities and inaccurate/incomplete documentation using your own data.
And for those of you who use RUG 44 or 66, we have great news you too – development is on schedule for these reports to be available October 1, 2010. As we make the successful transition to MDS 3.0, our comprehensive CareWatch solution includes these reports so that you can use your own data to see the potential impact of inaccurate or incomplete documentation that may result in missed service opportunities for your residents and missed reimbursement for your facility.
These tools are designed to match documentation with your clinical record and highlight possible inconsistencies which may affect both your care planning and RUG scores. Because of the glimpse “into the future” that these reports can provide, we use the term “potential” to describe them, which explains the titles, “Time-Weighted Resident RUG 34 Potential” and “Point in Time RUG 34 Potential.”
